The Arthur Nagel Community Clinic is committed to safeguarding the personal information entrusted to us by our patients and donors. We manage your personal information with the utmost concern for your privacy. This policy outlines the principles and practices we follow in protecting your personal information.  This policy applies to the Arthur Nagel Community Clinic and to any person or company providing services on our behalf.  A copy of this policy is provided to anyone upon request.

What is personal information?

Personal information means information about an individual. This includes an individual’s name, home address and phone number, age, sex, marital or family status, an identifying number, financial information, and educational history.

What personal information do we collect?

The clinic staff collects only the personal information that we need for the purposes of providing services to our patients and donors, including personal information needed to:

The clinic staff normally collects personal information directly from our patients and donors.  We may collect your information from other persons with your consent or as authorized by law.  We inform our patients and donors before or at the time of collecting personal information of the purposes for which we are collecting the information.  The only time we do not provide this notification is when a patient or donor volunteers information for an obvious purpose (For example, producing a credit card to make a donation when the information will be used only to process the payment).

Sharing and Usage

The clinic will never share, sell, or rent personal information with others for their promotional use. Information submitted to us is only available to clinic employees managing this information for purposes of contacting you or sending you emails based on your request for information, and to contracted service providers for purposes of providing services requested by you.

Why did you receive an email from us?

Our email marketing is permission-based. If you received a mailing from us, our records indicate that (a) you have expressly shared your email address for the purpose of receiving information in the future ("opt-in"), or (b) you have an existing relationship with us. We respect your time and attention by controlling the frequency of our mailings.  If you believe you have received an unwanted, unsolicited email sent via this system or purporting to be sent via this system, please contact ExecutiveDirector@NagelClinic.org.
