ELIGIBILITY FOR SERVICE
Patient's household income must not exceed 200 % of the Federal Poverty Guidelines (http://aspe.hhs.gov/poverty/08poverty.shtml) and have no medical insurance to qualify for service. A current tax form and pay stub are required documentation to determine eligibility for obtaining services. Proof of dependents claimed is also required.
